1. Navigate to the Surveys Tab
2. Click the Enable Survey checkbox
3. Enter your survey title and questions
4. Choose the type of question
a. No checkboxes – open ended question
b. Email – enters email automatically into sharing screen
c. Y/N – Yes or No buttons
d. A/B/C – Enable multiple choice questions. Click the icon to program the answers.
